Job Description

Center for Human Development, Inc. is seeking a highly skilled and compassionate Therapist and Coordinator to join our team in providing exceptional mental health services to individuals in need. As a Jail Diversion Program Coordinator and Therapist, you will work collaboratively with adults with a variety of diagnoses who are in jail or have recently been released. Your primary goal will be to help individuals maintain safely in the community by meeting with clients in the jail setting and at CHD.

Key Responsibilities:

  • Provide individual, family, and group therapy sessions to clients in the jail setting and at CHD.
  • Develop and implement individualized treatment plans to meet the unique needs of each client.
  • Collaborate with jail staff, community partners, and other healthcare professionals to ensure seamless care coordination.
  • Conduct assessments and screenings to identify client needs and develop effective treatment plans.
  • Participate in ongoing program development and evaluation to ensure the highest quality of care.

Requirements:

  • Masters degree in a social science field required.
  • LCSW preferred or willingness to pursue.
  • CADC or willingness to pursue certification.
  • Experience in MRT, forensic community-based collaborative treatment desirable.
  • Excellent communication and interpersonal skills.
  • Knowledge of assessment and treatment planning.
  • Computer literacy.
  • Ability to work in a team environment.
  • Willingness to perform occasional on-call crisis duties.
