Job Summary:

We are seeking a Healthcare Navigator to join our team at the North Colorado Health Alliance.

About the Role:

The Healthcare Navigator will work directly with individuals and families to provide guidance and support in navigating the healthcare system. This includes connecting them with medical and behavioral health treatment plans, scheduling appointments, and assisting with referrals and medication refills.

Responsibilities:

  • Enroll individuals in services using appropriate assessment tools and complete GPRA Surveys at enrollment and 6-month follow-up.
  • Support individuals in engaging with both medical and behavioral health treatment plans and assist with scheduling appointments, follow-up care, referrals, and medication refills.
  • Provide health needs assessments and identify risk factors to develop healthcare plans.
  • Enter all documentation, contacts, and assessments into required databases within the same or next business day.
  • Participate in telephone contacts, office/hospital visits, community visits, and other means of connecting with members to reach identified goals.
  • Perform health needs assessments and identify risk factors to develop healthcare plans.
  • Assist members in scheduling appointments, follow-up care, referrals, medication refills, etc.
  • Attend member appointments with providers and community resources as needed.
  • Work in collaboration, communication, and coordination among all responsible parties of an individual's multidisciplinary team to support achieving goals and reduce duplication of services.
  • Optimize member and family independence through education, community resources, and support.
  • Review cost-effective care and decrease duplication of services for members.
  • Understand, communicate, and facilitate on behalf of patients with regard to complaints, grievances, and appeal processes.
  • Regular case review with Supervisors and Trainers.
  • Maintain strict confidentiality of member records and communications.
  • Completion of Certified Addiction Technician Classes and attend supervision as required.
  • Safely transport members to and from MAT appointments in NCHA company vehicle.

Requirements:

  • High School Graduate or GED required.
  • Valid Colorado Driver License and Insurance required.
  • Bilingual preferred.
  • Reliable transportation.
  • Frequent contact with the public by phone and in person.

Salary: $55,000 - $65,000 per year.
