Medical Social Worker

We are seeking a skilled Medical Social Worker to join our team at Amazing Care Home Health Services. As a Medical Social Worker, you will play a vital role in providing support to patients and their families, empowering them to navigate the healthcare system and access local community resources.
Key Responsibilities:- Conduct thorough assessments of patients' psychosocial status and communicate findings to the case manager.
- Develop and implement social evaluations and intervention plans based on evaluation findings.
- Maintain accurate and up-to-date clinical records for all patients referred to social work services.
- Provide information and referral services to patients and families/caregivers regarding practical and environmental needs.
- Serve as a liaison between patients and families/caregivers and community agencies.
- Maintain collaborative relationships with agency personnel to support patient care.
- Develop and maintain contracts with public and private agencies as resources for patients.
- Participate in agency training programs and contribute to the development of the plan of care and case conferences as required.
- Assist physicians and other team members in understanding significant social and emotional factors related to health problems.
- Participate in discharge planning and ensure a smooth transition for patients.
- Master's degree in social work from a Council on Social Work Education-accredited program.
- Minimum one year of experience in healthcare, preferably in home health care.
- Excellent verbal and written communication, and organizational skills.
- Current CPR certification and a valid driver's license with a reliable vehicle.
