Position Overview

This role requires adherence to the Code of Ethical Conduct and fostering positive relationships within the organization, across departments, and with external partners.

The Home Health Speech Therapist provides specialized services to clients experiencing speech and language challenges.

Therapy is delivered under the guidance of the attending physician, with active involvement from family members, nursing staff, or other responsible individuals as necessary.

Key Responsibilities

  • Contributes to the formulation of individualized speech therapy care plans for patients.
  • Conducts patient visits for assessment and evaluation, ensuring communication with the physician prior to initiating care and as required, while completing necessary OASIS assessments.
  • Offers education and guidance to team members, patients, and their families.
  • Documents care provided and submits relevant paperwork promptly to maintain accurate medical records.
  • May engage in agency quality improvement initiatives.
  • Promotes the company's growth and sustainability through responsible resource management and community education regarding services offered.
  • Accepts additional assignments as deemed appropriate.
  • Adheres to agency policies and procedures, as well as state and federal regulations governing care delivery.
  • Receives direction from the Director of Clinical Services willingly.
  • Meets or surpasses established productivity benchmarks.
  • Delivers speech/language pathology services in alignment with the interdisciplinary care plan, recommending alternative communication methods and executing speech and swallowing exercises.
  • Participates in scheduled interdisciplinary team meetings to coordinate patient and family care, share information, problem-solve, and receive support and education.
  • Evaluates treatment outcomes and plans for discharge as necessary.
  • Supervises Home Health Aides when appropriate.
  • Ensures all care respects patient rights.
  • Reports any grievances or complaints from patients or families to the relevant authorities.
  • Reports any allegations of patient abuse or misappropriation of property.
  • Follows established safety and infection control protocols.
  • Identifies and responds to emergency situations appropriately.
  • Educates staff on psychosocial aspects as needed.
  • Attends required in-services and meetings.
  • Collaborates with service providers delivering contracted services to patients as necessary.
  • Participates in the development and revision of policies and procedures as requested.
  • Maintains accurate documentation for billing purposes.
  • Engages with patients, families, and staff to ensure satisfaction with services.
  • Communicates effectively with patients, families, and other healthcare professionals.
  • Performs additional duties as assigned.
Qualifications

  • Bachelor's degree in Speech Therapy is required.
  • Master's degree in Speech Therapy is preferred.
  • A minimum of one (1) year of experience in a home health environment is required.
Skills

Mathematical Skills:

Ability to perform basic arithmetic operations, including addition, subtraction, multiplication, and division using whole numbers, fractions, and decimals. Proficiency in calculating rates, ratios, and percentages.


Language Skills:
Ability to read, analyze, and interpret various professional documents, including business periodicals and governmental regulations. Proficient in writing reports, business correspondence, and procedural manuals. Capable of effectively presenting information and responding to inquiries from various stakeholders. Strong written and verbal communication skills are essential.

Other Skills and Abilities:
Proficient in understanding, reading, writing, and speaking English. Embraces and articulates the philosophy of hospice care.

Certifications, Licenses, and Registrations

Active and unencumbered Speech Therapist license in the state(s) of employment is required.

Physical Demands and Work Environment
The role requires the ability to perform essential functions effectively. Accommodations can be made for team members with disabilities.

Regular standing, walking, and manual dexterity are necessary, along with the capacity to lift and move objects weighing up to 50 pounds.

Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus.

In a healthcare setting, exposure to bodily fluids, infectious diseases, and typical conditions of the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential.
