Job Description

This Care Coordinator role involves working closely with clients, clinical staff, and stakeholders to develop and implement comprehensive care plans. The ideal candidate will possess excellent communication, problem-solving, and organizational skills, with a strong focus on delivering high-quality services.


Key Responsibilities
  • Engage clients in the BHCP program, ensuring their health and wellness needs are met.
  • Collaborate with clinical staff to develop person-centered treatment plans.
  • Provide health and wellness coaching, connecting clients to community resources and supports.
  • Maintain accurate documentation, adhering to HIPAA regulations and confidentiality requirements.

What We Offer

Requirements
  • BA/BS degree in human services or equivalent experience.
  • Experience as a Care Coordinator in a Behavioral Health Community Partner Program.
  • Strong communication, collaboration, and customer service skills.
  • Ability to maintain personal and professional boundaries.
