The Lighthouse Community Hospice Inc is seeking a compassionate and skilled Hospice Aide to join our team. As a Hospice Aide, you will play a vital role in providing high-quality care to patients and their families in their homes.
Key Responsibilities:- Provide personal care services, including bathing, dressing, and grooming, as assigned.
- Assist patients with daily living activities, such as meal preparation and ambulation.
- Monitor patients' conditions and report any changes to the Case Manager.
- Document patient care and observations in a timely and accurate manner.
- Collaborate with the interdisciplinary team to develop and implement patient care plans.
- Meet the training requirements of the State.
- Completed a minimum of 75 hours of classroom and supervised practical training.
- At least 18 years of age.
- Ability to read and follow written instructions and document care given.
- Understands hospice philosophy and is comfortable providing specialized care to the terminally ill.
The Hospice Aide will work in a community home environment, providing care to patients in their homes. The ability to work independently and make sound judgments is essential.
