Position Overview

The Healthcare Program Nurse is responsible for overseeing the medical needs of residents and providing nursing support within the MY Life and GADOL initiatives. Key duties include managing resident medical documentation, monitoring health conditions, and supervising the medication administration conducted by Authorized Direct Support Professionals (ADSPs).

Core Responsibilities

  • Facilitate training for medication administration staff, which encompasses an 8-hour medication class, annual evaluations, competency assessments, and additional training as required.
  • Perform comprehensive nursing assessments, evaluating physical health, mental well-being, medical history, and medication prescriptions on an annual basis or more frequently if necessary.
  • Administer the Health Risk Screening Tool annually for each resident.
  • Evaluate residents' ability to self-administer medications at least once a year, utilizing the SAMA assessment tool.
  • Ensure that each resident's health has been evaluated by a physician, advanced practice nurse, or physician assistant at least once a year.
  • Receive and inspect all medications, ensuring that labeling and orders contain all essential information, in accordance with regulatory standards.
  • Review medication administration records monthly for accuracy and completeness, including controlled substance documentation.
  • Maintain a detailed inventory and record of controlled substance usage.
  • Conduct random medication administration observations of each ADSP quarterly and provide re-training as necessary.
  • Inspect medications at each residence quarterly for expiration dates.
  • Analyze medication error reports monthly and collaborate with management to formulate corrective action plans.
  • Stay informed and oversee the implementation of all requirements outlined in relevant regulations.

Qualifications

  • Must possess a Registered Nurse (RN) license valid in the state of Illinois.
  • Completion of Nurse Trainer certification as mandated by relevant authorities.
  • A minimum of two years of nursing experience in a CILA facility or a comparable role.
  • Familiarity with relevant regulations.
  • Unblemished nursing and background record.
  • Strong verbal and written communication skills.
  • Proficient in medication management.
  • Ability to engage positively with clients and team members.
  • Valid driver's license with a clean driving record.
