Job Overview

Under the guidance of the Director of Nursing, the Healthcare Services Manager will oversee the coordination of health and wellness programs, along with personal care services for residents in assisted living or adult care facilities.

This role entails conducting clinical evaluations, supervising, and executing the approved individualized service plans developed collaboratively with residents, families, and healthcare providers to ensure the safety and well-being of residents within the facility.

The position requires flexibility in scheduling, with shifts available from Sunday to Thursday or Tuesday to Saturday, typically from 10:00 AM to 6:00 PM.

Qualified candidates may be eligible for a sign-on bonus.

The Healthcare Services Manager will ensure that care plans are tailored to meet the healthcare needs of residents, regularly reassessing clinical evaluations and adjusting plans as necessary. This role also includes providing oversight and supervision to nursing staff and home health aides from contracted agencies.

Key Responsibilities:

  • Manage the implementation of health-related services within the assisted living and adult care environments.
  • Engage in the assessment and screening of potential residents to determine suitability in accordance with established policies and regulations.
  • Contribute to the development of policies and procedures, offering recommendations to the Assistant Director of Nursing and Director of Nursing as needed.
  • Conduct assessments and reassessments for residents, identifying their health needs and determining the appropriate level of care required.
  • Maintain accurate documentation regarding resident care and any changes in their status.
  • Assist in formulating and executing individualized service plans based on assessment findings and resident needs.

Applicants should possess a degree from an accredited nursing program (BSN) and have experience in staff supervision and medication management, preferably in assisted living or similar settings. Experience with assessments, care planning, and management is essential, along with a minimum of two years of experience in providing services to older adults or residents in various healthcare environments.

A current Registered Nurse license in New York State is required.
