We are seeking a skilled Licensed Practical Nurse (LPN) to join our team as a full-time Private Duty Care Specialist. As a Private Duty LPN, you will provide specialized care to patients in their homes, focusing on G-tube, trach, and vent management.
Responsibilities- Deliver Expert Care: Provide high-quality care to patients, including G-tube, trach, and vent management, medication administration, and vital sign monitoring.
- Communicate Effectively: Communicate clearly with doctors, patients, and families to ensure seamless care coordination.
- Document Patient Information: Accurately document patient information, including medical history, treatment plans, and progress notes.
- Collaborate with Healthcare Team: Work collaboratively with the healthcare team to ensure comprehensive patient care.
- Active LPN License: Possess an active LPN license in Tennessee.
- Experience: Have at least 1 year of nursing experience, preferably in home health or private duty.
- Clinical Skills: Demonstrate strong clinical and assessment abilities, with expertise in G-tube, trach, and vent care.
- Communication Skills: Possess excellent communication skills, with the ability to effectively communicate with patients, families, and healthcare professionals.
