Job Summary:

Adoration Home Health and Hospice is seeking a dedicated LPN to provide skilled overnight care for a patient in a home setting. This role involves working a stable, full-time night shift and making a real difference in your patient's life.

Key Responsibilities:

Perform selected nursing procedures within the scope of LPN practice
Report significant changes immediately
Administer medications as instructed and as ordered by the physician and instructed by the RN and document the patient's file accordingly
Attend and participate in multi-disciplinary patient case conferences, staff meetings and in-service education programs as assigned

Requirements:

Must have 1 year of nursing experience due to trach care needs
Previous private duty, home health, hospice, hospital or skilled nursing experience preferred
Familiarity with electronic health records
