Job Summary

We are seeking a highly motivated and detail-oriented Intake Specialist to join our team at Summit Oaks Hospital. As an Intake Specialist, you will play a critical role in ensuring that patients receive the best possible care by coordinating their intake process.

Responsibilities
  • Collect demographic and clinical information from patients to facilitate their care
  • Provide referrals to patients for services that are not available at our hospital
  • Interface with various teams and departments to ensure seamless patient care
  • Assist patients in meeting their bio-psychosocial needs
Requirements
  • Bachelor's Degree in a related field
  • 3 years of experience in Behavioral Health or a related field
  • Strong communication and screening skills
  • Ability to work independently and manage time effectively
