Role Summary:
The Registered Nurse (RN) is responsible for delivering nursing care to terminally ill patients in a hospice setting. This role involves providing support and guidance to families during home care situations and bereavement, while collaborating with the hospice team to ensure quality care.
Qualifications for the Registered Nurse - Hospice:
- Prior hospice experience is essential.
- Must hold a valid registered nurse license in the state of Ohio.
- A minimum of 2 years of nursing experience is required, with hospice experience preferred.
- Active patient interaction within the last three years is preferred.
- Ability to work autonomously and make timely, accurate decisions.
- Capability to respond effectively to crises outside of a hospital environment.
- Willingness to adapt to diverse social, cultural, and religious backgrounds.
- Conduct regular assessments of hospice patients' nursing needs.
- Provide dietary guidance as needed.
- Administer hospice nursing services, treatments, and preventive care.
- Initiate nursing procedures tailored to the patient's hospice care and safety.
- Monitor and report any unexpected changes in the patient's condition to the physician and interdisciplinary team.
- Educate and support patients and their families in providing care.
- Oversee and mentor other nursing staff.
- Collaborate with the interdisciplinary team to develop and update patient care plans.
- Perform specific nursing tasks as required, following physician's orders.
- Participate in team meetings and maintain accurate records as mandated by hospice regulations.
- Adhere to hospice policies and procedures, ensuring confidentiality and compliance with HIPAA.
- Communicate with supervisors regarding any challenges in meeting patient needs.
- Participate in the on-call system and provide coverage as needed.
