We are seeking a compassionate and skilled Hospice Care Specialist to join our team at CommonSail Investment Group. As a Hospice Care Specialist, you will play a vital role in providing high-quality patient care and support to individuals in need.
Key Responsibilities- Assist patients with daily living activities, such as bathing, dressing, and grooming.
- Provide emotional and psychological support to patients and their families.
- Collaborate with healthcare professionals to develop and implement personalized care plans.
- Monitor patients' vital signs and report any changes to the clinical team.
- Maintain a clean and safe environment for patients.
- Currently certified as a STNA in the State of Ohio.
- Minimum of 1 year of Hospice Aide experience in a healthcare or home care setting.
- CPR certification preferred, but not required.
- Current driver's license with current automobile insurance and reliable transportation.
- Excellent communication and interpersonal skills.
