About the Role

We are seeking an experienced Occupational Therapist to join our team at LHC Group, a leading provider of home health care services. As a key member of our interdisciplinary team, you will play a vital role in delivering high-quality patient care and promoting independence for our clients.

Job Summary

The Occupational Therapist is responsible for conducting comprehensive assessments of patients' functional abilities, developing personalized therapy plans, and implementing evidence-based interventions to achieve optimal outcomes. You will work closely with patients, families, and healthcare professionals to ensure seamless care coordination and effective communication.

Key Responsibilities
  • Evaluate patients' functional status, including activities of daily living, fine motor coordination, and adaptive equipment needs
  • Develop and implement individualized therapy plans that align with patients' goals and priorities
  • Provide occupational therapy interventions, including exercises, training, and education, to promote independence and mobility
  • Collaborate with multidisciplinary teams to coordinate care and address complex patient needs
Requirements

To be considered for this role, you must possess:

  • A current Occupational Therapy licensure in the state of Ohio
  • Current CPR certification
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
