Nurse Practitioner Position

Lyle Health is seeking a skilled Nurse Practitioner to join our team in Manteca, CA. As a key member of our Integrated Care team, you will play a vital role in delivering high-quality, cost-effective care to our dialysis patients.
Key Responsibilities:
- Provide coordinated care services between patients, families, physicians, and other care team members.
- Facilitate autonomous and collaborative practice to ensure seamless patient care.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with healthcare professionals to ensure comprehensive and coordinated care.
Requirements:
- Active California license (highly preferred).
- Experience as a Family Nurse Practitioner, Adult Nurse Practitioner, Hospitalist, or in Nephrology, internal medicine, cardiology, or Case Management.
- Proficiency in clinical assessment, diagnosis, planning, implementation, documentation, and evaluation of chronic patients.
- Ability to make house calls when necessary.
