At CareSTL Health, we are seeking a highly skilled Clinical Community Health Specialist to join our team. This role is responsible for providing exceptional patient care and working collaboratively with healthcare professionals to ensure the highest level of service.

Job Summary

The Clinical Community Health Specialist will be responsible for delivering high-quality patient care, developing health improvement programs, and instructing individuals and groups on health education topics. This role requires excellent communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

Key Responsibilities
  • Patient Care: Directs and supervises less skilled nursing/health care personnel, prepares patients for examinations and treatments, assesses patient needs, and develops individualized care plans.
  • Health Education: Instructs individuals and groups on health education topics, including disease prevention and health promotion.
  • Program Development: Develops and implements health improvement programs, including training instruction for auxiliary personnel and community stakeholders.
  • Infection Control: Directs and coordinates infection control programs, advising and consulting with staff about necessary precautions.
Requirements

To be successful in this role, you must have a Graduate degree from an accredited school of Nursing and valid registration as a Registered Nurse in the State of Missouri. You should also have 0-5 years of experience in a Community Health Center.

Benefits

CareSTL Health offers a competitive salary range of $60,000 - $80,000 per year, based on experience. Additionally, you will receive a full benefits package, including paid holidays and Public Service Loan Forgiveness opportunities.
