We are seeking a compassionate and experienced Pastoral/Spiritual Counselor to join our Hospice team at AdvisaCare. As a key member of our care team, you will provide spiritual support and care to patients and their families in our Ann Arbor and surrounding areas.
Key Responsibilities:
- Conduct spiritual assessments and develop individualized spiritual plans of care for patients and their families.
- Collaborate with local clergy and healthcare teams to provide comprehensive care.
- Prepare and maintain accurate clinical and progress notes.
- Participate in Interdisciplinary Group (IDG) conferences and bereavement support for staff.
- Contribute to the Hospice performance improvement program and maintain high levels of customer satisfaction.
- Update Electronic Medical Records accurately and efficiently.
Requirements:
- Graduate of an accredited seminary or school of theology or equivalent certification in pastoral ministry.
- Proven experience in working with patients and families who have experienced loss.
- Empathetic approach to patients, caregivers, and team members.
- Ability to adapt to patient needs and status.
- Critical thinking skills for continuous improvement of Hospice Services.
