Licensed Vocational Nurse IV

We are seeking a skilled Licensed Vocational Nurse IV to join our team at Alameda Hospital. As a key member of our healthcare team, you will provide high-quality care to our residents, working collaboratively with our Registered Nurses and other healthcare professionals to ensure the best possible outcomes.
Key Responsibilities- Collect and analyze data to identify resident needs and develop effective care plans.
- Implement care plans, prioritizing resident needs and ensuring continuity of care.
- Document resident progress and outcomes, providing regular updates to the Registered Nurse.
- Collaborate with the Registered Nurse to identify resident and Primary Support Person (PSP) needs, and participate in teaching resident/PSP education.
- Adhere to safety and legal requirements, and demonstrate awareness of unit structure standards, nursing policies, and administrative policies.
- Recognize own limitations and education needs, improving knowledge and skill through in-service and continuing education classes.
- Participate in unit goal formation and assist in projects to achieve goals, and participate in QA/PI activities at the unit level.
- Communicate effectively with co-workers, physicians, residents, and residents' PSP, formulating and maintaining effective working relationships.
- Demonstrate ability to organize workload and set priorities.
- Identify problems, seek out relevant data, and demonstrate problem-solving skills.
To be successful in this role, you will need:
- Graduate of an accredited vocational school of nursing.
- Minimum one year recent experience as a LVN in a Long Term Care or Sub-Acute setting.
- Current licensure in the State of California to practice vocational nursing.
- Basic Life Support (BLS) certification issued by American Heart Association.
- IV Certification from Board of Vocational Nursing and Psychiatric Technicians.
