Key Position Details:
The Senior Health Co-Care program is seeking a Nurse Practitioner to join our team covering the northeast metro region. This position is responsible for delivering primary care in the homes of medically complex, homebound patients.
Job Responsibilities:
- Provide Clinical Care:
- Conduct comprehensive patient assessments to determine diagnosis and treatment plans.
- Order and interpret medical diagnostics in collaboration with attending physicians.
- Prescribe medications, treatments, and medical equipment as needed.
- Provide patient education on managing chronic illnesses and wellness care.
- Refer patients to specialty care when necessary.
- Consult with physicians when patient needs exceed Nurse Practitioner scope of practice.
- Participate in Healthcare Team:
- Promote effective communication within the team and with other healthcare staff.
- Act as a resource to team members and provide support as needed.
- Collaborate with primary care teams for program development and implementation.
- Serve as a liaison with outside agencies as required.
- Other Duties:
- Perform other duties as assigned by management.
Required Qualifications:
- Master's degree in Nursing or related field from an accredited institution.
- At least one year of experience in primary care or senior health Advanced Practice.
Licenses/Certifications:
- Licensed Registered Nurse in the state of Minnesota.
- Certified Nurse Practitioner in the relevant specialty.
- ACLS-BLS Tier 2 certification within 180 days of hire.
- Drug Enforcement Administration certification upon hire.
- Valid driver's license in the state of residence.
Physical Demands:
Medium Work: Lifting weight up to 32 lbs. occasionally, up to 25 lbs. frequently.
