Key Position DetailsThe Senior Health Co-Care program is seeking a skilled Nurse Practitioner to join our team, providing primary care to medically complex, homebound patients in the northeast metro region.
Job DescriptionAs a Nurse Practitioner, you will be responsible for delivering comprehensive patient care, including diagnosis, treatment, and consultations as part of a physician/nurse team. Your key responsibilities will include:
- Providing clinical care for patients, including comprehensive assessments, diagnosis, and treatment planning.
- Participating in the healthcare team, promoting communication and collaboration with interdisciplinary staff.
- Consulting with physicians and other healthcare professionals as needed.
- Conducting patient education and referrals for specialty care when necessary.
To be successful in this role, you will need:
- A Master's degree in Nursing or a related field.
- 1 or more years of experience in primary care or senior health Advanced Practice.
- Current licensure as a Registered Nurse in Minnesota and certification as a Nurse Practitioner in your specialty.
You will be required to obtain the following certifications and licenses:
- Licensed Registered Nurse - MN Board of Nursing.
- Certified Nurse Practitioner - American Nurse Credentialing Center (ANCC) certification.
- ACLS-BLS Tier 2 - Multisource.
- Drug Enforcement Administration.
This role requires medium work, with occasional lifting of up to 32 pounds and frequent lifting of up to 25 pounds.
