Job SummaryWe are seeking a skilled Physical Therapist to join our team, providing physical therapy services to patients in their homes. As a Physical Therapist with HarmonyCares, you will have the opportunity to make a meaningful difference in the lives of our patients, while enjoying a rewarding and challenging career.
Responsibilities- Provide comprehensive physical therapy services, including assessment, evaluation, and treatment planning, to patients in their homes.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Develop and implement individualized patient care plans, incorporating the patient's goals and preferences.
- Collaborate with other healthcare professionals to ensure comprehensive care and optimal patient outcomes.
- Document patient progress and outcomes, using standardized assessment tools and electronic health records.
- Current, unencumbered State Physical Therapy License.
- One year of experience as a home care professional Physical Therapist, with a strong background in comprehensive assessment and OASIS-C.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Proficiency in clinical skills, including assessment, evaluation, and treatment planning.
- Ability to drive in a car 2-4 hours per day, with a valid driver's license and good driving record.
