Job Summary

We are seeking a skilled Physical Therapist to join our team of healthcare professionals. As a Physical Therapist with HarmonyCares, you will have the opportunity to make a meaningful difference in the lives of our patients, providing personalized care and support to help them achieve their goals.

Responsibilities
  • Assess and evaluate patients to develop personalized care plans
  • Provide physical therapy services, including exercises, stretches, and other interventions to improve mobility and function
  • Communicate with patients, families, and healthcare teams to ensure seamless care coordination
  • Supervise and instruct non-professional home health aide staff in the provision of personal care to patients
  • Update care plans and submit visit notes as required
  • Attend staff meetings and educational in-services to stay current with best practices
Requirements
  • Current, unencumbered State Physical Therapy License
  • Current driver's license, good driving record, proof of insurance, and reliable mode of transportation
  • One year experience as a home care professional Physical Therapist and competent in performing comprehensive assessment and OASIS-C preferred
  • Ability to make sound professional clinical judgment and assess patient needs
  • Proficiency in clinical skills and Microsoft Office
