We are seeking a skilled Case Manager to join our team at Maritime Behavioral Health Management. As a Case Manager, you will play a critical role in providing high-quality care to our clients and ensuring their successful transition to appropriate levels of care.
Key Responsibilities- Develop and implement discharge/transfer plans to ensure seamless movement to the next level of care.
- Collaborate with referral sources, including mental health treatment providers, criminal justice agencies, schools, and employers, to provide case consultation and coordination.
- Coordinate ongoing care for client housing and therapeutic needs with our treatment team, substance abuse treatment providers, healthcare providers, community resources, and family members.
- Participate as a member of a multidisciplinary team to provide comprehensive care to assigned clients.
- Document all counseling and education services, assessments, reassessments, referrals, and follow-up in the client's clinical record.
- Promote Windward Treatment to prospective clients and employees.
- Orient new clients to the facility and PHP/IOP program.
- Facilitate communication with ancillary services.
- Attend clinical and administrative meetings to integrate inter-departmental sharing of information.
- Build positive relationships with clients and demonstrate adaptability and flexibility in a dynamic work environment.
