Job Title: Licensed Practical Nurse

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at IntelyCare. As an LPN, you will work in post-acute facilities in the Braintree, MA area, providing high-quality patient care and support to residents.

Job Responsibilities:
  • Provide direct patient care, including administering medications, monitoring vital signs, and conducting intakes.
  • Develop and implement individualized care plans to meet the unique needs of each resident.
  • Collaborate with healthcare teams to ensure seamless care transitions and effective communication.
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments.
Qualifications:
  • Current Massachusetts state license to practice as a Licensed Practical Nurse (LPN).
  • Completion of approved LPN educational program.
  • High school diploma or equivalent.
  • Ability to lift a minimum of 50 pounds.
  • Minimum of 6 months of experience in a related role.
