Medical Care Coordinator

We are seeking a highly skilled and compassionate Care Manager to join our team at Passionate Care Management. As a Care Manager, you will play a vital role in assisting doctors in continuing communication with their patients. Your primary responsibility will be to act as a liaison between patients and their providers, handling national tele-med calls to patients and creating a patient-centered, comprehensive plan of care for all health issues.
Key Responsibilities:
- Verify and record patient demographics, medical issues, medications, goals, barriers, and allergies to create a clinical summary using our certified software program.
- Create a patient-centered, comprehensive plan of care for all health issues based on a physical, mental, cognitive, psychosocial, functional, and environmental (re)assessment, and an inventory of resources.
- Help provide patients with a means to make timely contact with healthcare practitioners in the practice who can address any urgent chronic care needs. Ensure continuity of care with their designated practitioner by setting and reminding of regularly scheduled visits.
Requirements:
- Must be currently certified: CNA, PCT, CMA, CCMA, or RMA.
- Ability to follow HIPAA guidelines.
- Handle potentially difficult conversations in a calm and professional manner.
- Previous F2F healthcare experience required.
- Previous phone experience preferred.
- Compensation range $18-$26 per hour with incentive.
