We are seeking a compassionate Nurse Practitioner to join our team at Abode Care Partners. As a leading provider of integrated medical services, we care for individuals from post hospitalization to home in various settings.
Job SummaryThe Nurse Practitioner will provide personalized medical care to seniors and individuals with complex needs, focusing on evidence-based practices. About the RoleThis position involves examining patients, ordering diagnostic tests, collaborating with healthcare professionals, and maintaining patient records. The successful candidate will have a strong knowledge base of HIPAA and OSHA regulations and be proficient in medical instruments and equipment.
Key Responsibilities- Perform evidence-based medical care by planning, directing, and evaluating patient medical needs
- Examine and treat chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses
- Order and interpret laboratory and diagnostic testing as needed
- Collaborate with caregivers and other healthcare professionals in providing interdisciplinary care and maintaining a comprehensive plan of care
- Provide referrals to other healthcare practitioners when necessary
- Maintain up-to-date patient medical records
- Active state Nurse Practitioners license
- Current Nurse Practitioner National Board Certification
- Active state DEA licensure
- Master's degree in Nursing (MSN)
- Five or more years of nursing experience; at least two years of nurse practitioner experience preferred
- Strong interpersonal communication skills – verbal and written
- Excellent time management skills with accountability for practitioner's daily workflow
- Licensed driver with valid automobile insurance
- Ability to engage in moderate physical activity
