Key Responsibilities for Hospice Registered Nurses:
- Conduct an initial evaluation of the patient and family to identify hospice requirements
- Formulate a care plan that sets objectives based on nursing diagnoses and integrates palliative nursing interventions
- Facilitate coordination among the clinical team supporting the patient, while overseeing LPNs and Hospice aides involved in patient care
- Engage in meaningful interactions with patients to assist with pain management and emotional support
- Adjust and reassess the care plan as needed
- Monitor, document, and communicate symptoms and variations in the patient's condition
- Acquire and process physician orders as necessary
- Participate in on-call responsibilities as outlined by the on-call policy
- Perform additional duties as assigned
Qualifications for Hospice Registered Nurses:
- Must possess a current Registered Nurse license to practice in the Commonwealth of Virginia
- A minimum of one year of hospice experience is required
- Graduate from an accredited nursing program
- Must hold a valid driver's license and have access to a reliable vehicle for work-related travel
