Job Summary

We are seeking a highly skilled and compassionate Intake Coordinator to join our team at VITAS Healthcare San Francisco Bay. As an Intake Coordinator, you will play a critical role in ensuring seamless patient access to our end-of-life care services.

Key Responsibilities
  • Process New Referrals: Effectively manage new referrals, prioritizing tasks to meet the needs of referral sources and patients.
  • Collaborate with Admissions Team: Participate in daily check-in meetings to review staffing and scheduling needs, ensuring timely and efficient patient admissions.
  • Provide Exceptional Customer Service: Deliver a concierge-level of service to patients, referral sources, and internal stakeholders, adhering to VITAS Call Center Quality Management standards.
  • Support Admissions Goals: Contribute to the achievement of program goals by providing accurate and timely information, escalating matters as needed to ensure seamless patient admissions.
  • Establish Strong Relationships: Foster effective relationships with internal and external stakeholders, ensuring open communication and collaboration to drive patient access and satisfaction.
  • Manage Data Integrity: Accurately enter information into VITAS computer applications, maintaining data integrity and supporting business intelligence initiatives.
  • Respond to Patient and Referral Source Inquiries: Provide accurate and timely responses to patient and referral source inquiries, ensuring clear communication and resolution of concerns.
  • Coordinate Appointments: Schedule and confirm appointments with patients and referral sources, ensuring timely and efficient patient admissions.
  • Escalate Matters as Needed: Collaborate with Admissions Manager to resolve complex issues and ensure seamless patient admissions.
  • Participate in Ongoing Training and Development: Engage in ongoing training and development initiatives to stay current with changing healthcare regulations and best practices.
Requirements
  • Current CA LVN License: Possess a current California Licensed Vocational Nurse (LVN) license.
  • Related Experience: Two or more years of experience in medical terminology and/or a call center environment.
  • Ability to Work in a Fast-Paced Environment: Demonstrate the ability to work in a fast-paced, dynamic environment, prioritizing tasks and managing multiple responsibilities.
  • Excellent Communication and Interpersonal Skills: Possess excellent communication and interpersonal skills, with the ability to effectively interact with patients, referral sources, and internal stakeholders.
  • Working Knowledge of Computers and Software: Possess a working knowledge of computers, internet access, and the ability to navigate within automated systems and software packages.
  • Ability to Type 45 WPM: Type a minimum of 45 words per minute.
  • Prior Experience in Collaborative Leadership: Possess prior experience in working collaboratively with other functional leaders to drive action plans.
  • Highly Organized and Able to Manage Multiple Responsibilities: Demonstrate the ability to manage multiple responsibilities and work on various assignments simultaneously.
  • Model Positive Behaviors: Model positive behaviors that support the company's values, including integrity, valuing diversity, innovation, building relationships, customer focus, and teamwork.
  • Ability to Communicate Effectively with Department Heads, Managers, Coworkers, and Vendors: Possess the ability to communicate effectively with department heads, managers, coworkers, and vendors to resolve problems and negotiate resolutions.
Education
  • High School Diploma or Equivalent: Possess a high school diploma or basic education equivalency.
  • Bilingual in Spanish/English: Possess bilingual skills in Spanish and English.
  • Medical or Business Office Education or Training: Possess medical or business office education or training.
