Job Summary

We are seeking a highly skilled Licensed Clinical Social Worker to join our team at Lawrence Health Services. As a key member of our care management team, you will play a vital role in evaluating, planning, and implementing options and services to meet the health needs of our patients.

As a Licensed Clinical Social Worker, you will be responsible for providing direct care to clinic patients, including diagnostic assessment, individual therapy, family therapy, group therapy, and crisis intervention. You will work closely with our multi-disciplinary team to promote patient health outcomes and provide support to patients and families.

Key Responsibilities
  • Evaluate patient needs and develop individualized plans to meet those needs
  • Provide direct care to patients, including therapy and counseling
  • Collaborate with the treatment team to promote patient health outcomes
  • Assess and address the psychological, financial, and discharge planning needs of patients and families
  • Provide information, support, and referrals to other levels of care and appropriate resources
Requirements
  • Master's degree in Social Work
  • Licensed as a Social Worker
  • 4,000 hours of post-graduate, supervised social work experience
  • Excellent verbal and written communication skills
  • Computer skills for data collection and analysis
