Acting as an advocate for patients, the Clinical Pharmacist is involved in the planning and execution of pharmaceutical care, taking into account each patient's medical conditions and personal history. These responsibilities span all facets of pharmacy practice within the organization. The Clinical Pharmacist is legally accountable for reviewing and authorizing all actions related to the dispensing of patient-specific medications based on valid prescriptions or medication orders from licensed practitioners as outlined in relevant regulations.
Key Responsibilities
- Conduct thorough order verification
- Ensure precise dispensing of medications
- Reduce order processing time
- Monitor and document medication use
- Record clinical activities
- Perform and document medication reconciliation
- Manage inventory and comply with Hospital Formulary policies
- Follow local and organizational Policies and Procedures
- Execute other assigned tasks as needed
- Adhere to all policies and standards
- Refer to departmental documentation for specific duties during orientation
- Maintain the confidentiality and security of Protected Health Information (PHI) for patients, following annual training and organizational policies
Education
- Bachelor's Degree in Pharmacy (Required) or
- Pharm D degree (Required)
- Prior experience in the relevant practice area, particularly with computerized unit dose systems, sterile products, and patient-centered clinical skills (Preferred)
- Strong communication skills are essential (Required proficiency)
- Proficiency in computer systems and office equipment, including automated compounding systems (Required proficiency)
- Registered Pharmacist (R.Ph) in the State of Ohio (Required)
- Licensed health sciences practitioner in the State of Ohio (Required)
- Frequent standing and walking
- Occasional sitting
- Frequent lifting and carrying of up to 50 lbs
- Frequent pushing and pulling of up to 50 lbs
- Occasional climbing and balancing
- Frequent stooping, kneeling, crouching, and reaching
- Constant handling, grasping, feeling, talking, and hearing
- Constant eye/hand/foot coordination
- 10% travel may be required
