Assisted Living Team Member

We're seeking a dedicated Care Manager to join our team at Sunrise of Leominster, where you'll have the opportunity to make a meaningful difference in the lives of our seniors.
In this role, you'll be responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in our assisted living and reminiscence neighborhoods.
Your day-to-day responsibilities will include:
- Developing strong relationships with seniors and their families, and providing individualized care to meet their unique needs and preferences.
- Working collaboratively with management to develop and implement individualized service plans (ISPs) and monthly updates.
- Providing support and assistance with daily living activities, such as bathing, dressing, and grooming.
- Participating in life-enriching activities, such as arts and crafts, exercise classes, and outdoor events.
To be successful in this role, you'll need:
- A high school diploma/GED accepted, and may be required per state regulations.
- A minimum of 18 years of age and certification in relevant areas, if required by state regulations.
- Excellent communication and interpersonal skills, with the ability to work effectively with seniors, families, and colleagues.
- Ability to work independently and as part of a team, with a focus on delivering high-quality care and services.
- Previous experience working with seniors is preferred, but not required.
We offer a competitive salary, estimated at $45,000 - $65,000 annually, depending on location and qualifications. At Sunrise, we prioritize the well-being and satisfaction of our team members and residents, and we strive to create a positive and supportive work environment.
