Job Summary:

The Licensed Practical Nurse will provide high-quality patient care and support to our medical providers in a fast-paced family practice setting. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a team environment.

Responsibilities:
  • Provide routine patient care, including vital signs, height, weight, vision/hearing testing, and other procedures as directed by the supervising provider/Clinic Manager.
  • Accurately chart patient information in the medical record, following established SCC policies and procedures.
  • Assist nurses and providers as needed, maintaining a high level of professionalism and patient focus.
  • Maintain exam rooms and work areas, ensuring they are well-stocked, orderly, and clean, including disinfecting exam tables between patients.
  • Perform phlebotomy/fingerstick as permitted by training, administer injections according to position requirements and as trained, and perform procedures as ordered and trained.
  • Accurately make/cancel/reschedule appointments in the appointment system, follow-up on appointment to determine patient compliance and contact patients to reschedule (no-show policy).
  • Maintain strict infection control, including sterilizing instruments, performing procedures, needle disposal, and understanding emergency protocols and responding accordingly.
  • Have current knowledge of and participate in maintenance of drug assistance programs, understand and assist in the referral process of patients to outside services or providers, and follow-up on patient referrals as necessary.
  • Understand mandatory reporting of child/elder abuse policy and procedure, understand and utilize the vaccine program where applicable, and document accurately on forms and vaccine administration records.
  • Counsel patients per treatment plan, including diet, exercise, medication, and other assigned tasks, demonstrating knowledge and skills necessary to provide care appropriate to the age of the patients served, and provide culturally sensitive care.
  • Perform in-house waived testing, including pregnancy tests, hemocue/Hemoglobin, glucometer, urine dip, hemoccult-stool, oraquick, or throat swab, following directions/procedure and logging results in patients' medical records.
  • Conducts Point of Care Testing and quality controls, documents/processes patient requests and coordinates timely and appropriate responses, escalates requests to RNs and providers as appropriate, preps patient appointments by conducting chart review and proactively engaging patients as needed, and performs other duties as assigned.
Requirements:
  • Minimum one (1) year of Licensed Practical Nurse experience preferably in a family practice setting.
  • Minimum High School or General Education Diploma Equivalent.
  • Completion of an accredited Licensed Practical Nurse Program.
  • Active Licensed Practical Nurse Licensure.
  • Basic Life Support.
