Job Summary

SSM Health is seeking a skilled RN Case Manager to join our team. As a key member of our healthcare team, you will play a vital role in ensuring patients receive the best possible care and support during their hospital stay.

Responsibilities
  • Assess patients' discharge planning needs and develop individualized plans to ensure a safe and successful transition to the next level of care.
  • Collaborate with interdisciplinary teams to identify and address barriers to discharge, including social determinants of health and access to community resources.
  • Utilize evidence-based practices and clinical expertise to optimize patient outcomes and reduce readmissions.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
  • Participate in quality improvement initiatives to enhance the patient experience and improve healthcare outcomes.
Requirements
  • Graduate of an accredited nursing program.
  • Three years of acute care experience, preferably in a hospital setting.
  • Current RN licensure in the state of Illinois, Missouri, Oklahoma, or Wisconsin.
