Job Summary

As a RN Case Manager at SSM Health, you will play a vital role in ensuring patients receive seamless care transitions from hospital to home. This position requires a skilled and compassionate nurse to assess, develop, and implement discharge plans that meet the unique needs of each patient.

Key Responsibilities
  • Conduct thorough assessments of patients to identify potential barriers to discharge and develop plans to overcome them.
  • Collaborate with interdisciplinary teams to ensure patients receive comprehensive care and support.
  • Develop and implement discharge plans that meet the needs of patients and their families.
  • Monitor patient progress and make adjustments to discharge plans as needed.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements
  • Graduate of an accredited nursing program.
  • Current RN licensure in the state of Illinois, Missouri, Oklahoma, or Wisconsin.
  • Three years of acute care experience.
