Job Summary

As a Licensed Vocational Nurse II - Outpatient, you will assist in the planning and delivery of nursing services for patients under the direction and guidance of a Registered Nurse. You will utilize direct observation or electronic monitoring to identify patient needs and provide direct nursing care. You will also support unit functions and collaborate with the healthcare team to facilitate patient outcomes.

Key Responsibilities
  • Identify the impact of cultural diversity and lifestyle choices on patient condition and make accommodations for such differences.
  • Set priorities according to most emergent needs on an ongoing basis.
  • Perform LVN skills in compliance with all approved policies/procedures with minimal or indirect supervision.
  • Recognize and clarify inappropriate orders.
  • Collaborate with the healthcare team in a timely manner to facilitate patient outcomes.
  • Re-enforce learning and discharge planning needs of patient/family and document teaching in a timely manner.
  • Routinely take usual and special precautions relative to the age, medical history, condition of the patient, and the type of treatment being given.
  • Document clinical activities in a timely manner.
  • Documentation is legible, comprehensive, and accurate.
Requirements
  • Minimum of one (1) year experience in previous three (3) years as an LVN in an acute hospital, outpatient, or Home Health/Hospice setting or completion of New Graduate LVN training program.
  • Vocational Nurse License (California)
  • I.V. Certification within three (3) months of hire
  • Basic Life Support
Physical Demands

The physical demands of this position include heavy physical demands, frequent pulling, pushing, and lifting of patients and equipment, and walking and standing most of the time on duty.
