Springwell is seeking a skilled Case Manager to join our interdisciplinary team. As a Case Manager, you will play a vital role in ensuring that older adults receive the necessary support and services to live at home with dignity and independence.
Key Responsibilities- Conduct comprehensive assessments of older adults to determine their needs and eligibility for in-home services.
- Develop and implement personalized care plans, coordinating services with a network of vendor agencies as part of an interdisciplinary team.
- Communicate and consult with family members, other Springwell staff, and community professionals to ensure seamless service delivery.
- Enter data into an electronic statewide database and maintain accurate records.
- Bachelor's Degree or higher in a related field.
- Applicants without a Bachelor's Degree may be considered if they have an Associate's Degree and at least 3 years' experience working with elders or providing case management services.
- Experience working with computers, entering data, and running reports.
- Reliable transportation, current driver's license, and safe driving record.
- Generous paid time off, including 3 weeks of vacation in the first year.
