Home Care Physical Therapy Assistant PTA

At Liberty Home Care, we're dedicated to providing recovery with independence to our patients. As a Home Care Physical Therapy Assistant, you'll play a vital role in delivering patient care services and direct treatment as assigned by the Physical Therapist.
Key Responsibilities:- Update the care plan in coordination with the therapist and physician, and plan with the patient/family toward adjustment.
- Document patient care data accurately, thoroughly, and in a timely manner, reflecting evidence of improvement/progress/decline in patient's condition.
- Coordinate with Patient Care Coordinator in making referrals to appropriate disciplines.
- Attend and participate in patient care conferences, meetings, and mandatory in-services as scheduled. Read the minutes of patient care conferences not attended.
Requirements:
- Completion of an accredited physical therapy assistant program and current State licensure.
- Minimum of one year's therapeutic experience in a non-home health care environment; strong knowledge of theory and concepts relating to the practice of physical therapy and the ability to treat patients for whom a physician's order exists.
- Must accept travel and working conditions as assigned.
