Job Title: LVN Charge Nurse

Regency Integrated Health Services is seeking a dedicated and compassionate LVN Charge Nurse to join our team at the Town and Country Nursing and Rehabilitation Center in Boerne, TX.

Job Summary:

The LVN Charge Nurse will play a vital role in ensuring the provision of high-quality care to our residents. This includes assisting in the development and implementation of individualized care plans, administering medications, and conducting rounds with physicians to ensure the best possible outcomes for our residents.

Key Responsibilities:
  • Observe, assess, document, and report resident condition/changes during rounds
  • Assure implementation of care plans as directed by physicians and facility policies
  • Administer medications as prescribed and perform narcotic reconciliation with incoming and outgoing shifts
  • Conduct rounds with physicians and record visits in clinical records
  • Identify significant changes in resident condition and take necessary action per facility policies and procedures
  • Oversee and monitor aides to ensure the highest level of care
  • Initiate and complete Incident and Accident Reports per facility policy
  • Discharge residents per facility policy
  • Communicate with resident's point of contact regarding status updates or changes in condition
Qualifications:
  • Active Licensed Vocational Nurse in the state of Texas in good standing
  • Completed an LVN program at an accredited vocational school or college
  • Experience working in a skilled nursing facility is preferred
