Job Summary:

The LVN Charge Nurse will play a vital role in ensuring the provision of quality care to our residents. This includes observing, assessing, and documenting resident conditions, implementing care plans, administering medications, and conducting rounds with physicians.

Key Responsibilities:
  • Observe and assess resident conditions, documenting and reporting changes
  • Implement care plans as directed by physicians and nursing staff
  • Administer medications as prescribed by physicians
  • Conduct rounds with physicians and record visits in clinical records
  • Perform narcotic reconciliation with incoming and outgoing shifts
  • Identify significant changes in resident conditions and take necessary action
  • Oversee and monitor aides
  • Initiate and complete Incident and Accident Reports
  • Discharge residents as per facility policy
  • Communicate with resident's point of contact regarding status updates or changes in condition
Requirements:
  • Active Licensed Vocational Nurse in the state of Texas in good standing
  • Experience working in a skilled nursing facility preferred
