About the Role:

We are looking for a Physical Therapist to provide high-quality care to adult and geriatric patients in their homes. As a member of our multidisciplinary healthcare team, you will work closely with nurses and other healthcare professionals to develop and implement individualized care plans.

Responsibilities:
  • Conduct home visits to assess patients' physical needs and develop personalized care plans
  • Perform diagnostic tests and measurements to evaluate patients' mobility, strength, and balance
  • Develop and implement manual therapeutic exercises, gait training, and other interventions to improve patients' physical function
  • Collaborate with nurses and other healthcare professionals to revise and update care plans as needed
  • Educate patients and their families on rehabilitative care and activities to promote independent living
Qualifications:
  • Current license as a Physical Therapist in Massachusetts
