Key Responsibilities:
  • Collaborate with various disciplines to develop and implement effective care plans.
  • Communicate effectively with patients, families, and healthcare professionals.

Requirements:
  • Bachelor's degree in Social Work or a related field (Social Work experience preferred).
  • Basic knowledge of Medicaid process required.
  • Able to work effectively in an individual and team setting.
