About the Role:

We are seeking a highly skilled and experienced Admissions Registered Nurse to join our team. As an Admissions RN, you will be responsible for completing admissions and/or visits for our Home Care team, working independently in the field to visit patients one on one in their home setting.

Key Responsibilities:
  • Completing admissions and/or visits for our Home Care team
  • Working independently in the field to visit patients one on one in their home setting
  • Providing exceptional care and compassion to patients and their families
  • Collaborating with our interdisciplinary team to ensure seamless care delivery
  • Meeting productivity and quality standards
Requirements:
  • Graduate of an accredited school for RN and licensed to practice as a Registered Nurse in the state of Minnesota
  • Minimum of twelve (12) months experience working in an acute care or home health environment
  • Evidence of current CPR certification
  • Experience in Home Care to include OASIS; minimum one (1) year experience preferred
  • Able to travel within a geographic area serviced by the office
