Medical Care Specialist

We are seeking a highly skilled Nurse Practitioner (NP) to join our team at Abode Care Partners, a leading provider of integrated medical services. As a key member of our healthcare team, you will play a vital role in delivering high-quality, patient-centered care to seniors and individuals with complex needs.
Responsibilities- Perform comprehensive medical assessments, develop, and implement personalized care plans to address chronic and urgent health issues.
- Collaborate with multidisciplinary teams to ensure seamless delivery of care, including regular communication with caregivers and other healthcare professionals.
- Order and interpret laboratory and diagnostic testing as needed to inform treatment decisions.
- Provide education and support to patients and their families on disease management, self-care techniques, and lifestyle modifications to improve overall health outcomes.
- Maintain accurate and up-to-date patient records, adhering to strict confidentiality and HIPAA guidelines.
- Current state licensure as a Nurse Practitioner (NP) and active DEA licensure.
- Master's degree in Nursing (MSN) from an accredited institution.
- A minimum of five years of nursing experience, with at least two years of NP experience preferred.
- Excellent interpersonal and communication skills, with the ability to work effectively with diverse patient populations and multidisciplinary teams.
- Strong knowledge of OSHA and HIPAA regulations, as well as proficiency in electronic medical record (EMR) systems and Microsoft Office applications.
