End-of-Life Care Specialist: Join Gentiva Hospice Today

Gentiva Hospice is seeking a skilled Nurse Practitioner to join our team as an End-of-Life Care Nurse Practitioner. In this role, you will work closely with our multidisciplinary team to provide high-quality care to patients and their families.

About the Position

This is a flex/per diem position that requires a Master's or higher degree in Nursing, completion of a graduate-level nurse practitioner education program accredited by a national accrediting body, and current state licenses as a registered nurse and advanced practice nurse.

Responsibilities

  • Conduct physical assessments within regulatory time frames established by CMS and provide recommendations to the IDG.
  • Communicate clinical findings to physicians responsible for certifying or recertifying hospice eligibility.
  • Maintain accurate records and notify the agency when unable to complete scheduled visits.

Salary and Benefits

We offer a competitive salary range of $140,000 - $170,000 per year, depending on experience, along with a comprehensive benefits package including health insurance, 401(k) plan, tuition reimbursement, and paid time off.
