About the Role

We are seeking a highly skilled and compassionate Nurse Practitioner or Physician Assistant to join our Elder Care team at Essentia Health. As a key member of our interdisciplinary team, you will provide exceptional healthcare to our adult patient population, emphasizing health promotion and disease prevention.

Responsibilities
  • Provide comprehensive health assessments, diagnosis, and treatment to patients in a variety of settings, including nursing homes and assisted living facilities.
  • Collaborate with physicians and other healthcare professionals to ensure seamless care coordination and patient outcomes.
  • Develop and implement individualized care plans, taking into account patients' unique needs and goals.
  • Stay up-to-date with the latest evidence-based practices and guidelines in elder care.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.
Requirements
  • Master's or Doctorate degree in Nurse Practitioner or Bachelor's/Master's degree in Physician Assistant program from an accredited institution.
  • Current licensure as a registered nurse in the appropriate state (if NP).
  • Current licensure as a Nurse Practitioner or Physician Assistant in the appropriate state.
  • Current certification in CPR.
  • National certification, in applicable area.
  • Successful completion of Essentia Health credentialing process prior to practice.
