Job Title: Home Health Physical Therapist

LHC Group is seeking a skilled Home Health Physical Therapist to join our team in East Berlin, Connecticut.

About the Role:

We are looking for a compassionate and dedicated Physical Therapist to provide high-quality patient care in the comfort of their own homes. As a Home Health Physical Therapist, you will be responsible for assessing and evaluating patient care needs, developing treatment plans, and implementing interventions to improve patient outcomes.

Responsibilities:
  • Conduct thorough assessments and evaluations of patient care needs
  • Develop and implement individualized treatment plans to meet patient goals
  • Provide skilled physical therapy interventions to improve patient mobility, strength, and function
  • Collaborate with interdisciplinary teams to ensure comprehensive patient care
  • Document patient progress and outcomes in accordance with regulatory requirements
Requirements:
  • Current licensure to practice physical therapy in the state of Connecticut
  • Current CPR certification
  • Access to a dependable vehicle or public transportation
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
