Job Overview:

The Licensed Physical Therapist is tasked with delivering comprehensive rehabilitative services to clients both at the facility and in their homes. This role involves close collaboration with the medical team, including physicians and rehabilitation specialists, to enhance client independence and safety while working towards restoring functionality.

Key Responsibilities:

  • Conduct thorough assessments to identify the rehabilitation needs and objectives of clients, actively participating in interdisciplinary team meetings to formulate effective care plans.
  • Implement and document therapeutic interventions as per physician directives and agreed-upon care plans, which may include various treatment modalities such as exercise, muscle re-education, and functional training.
  • Oversee and mentor Therapy Assistants, providing necessary training and guidance for the execution of Functional Maintenance Programs.
  • Educate clients, families, and staff on the proper use and maintenance of therapeutic equipment.
  • Manage equipment procurement and maintenance, ensuring all tools are in optimal working condition.
  • Maintain open communication with team members and caregivers regarding any changes in client conditions or needs.
  • Facilitate smooth transitions of care across different settings, ensuring continuity and quality of service.
  • Support marketing efforts to attract new clients to the program.
  • Engage in end-of-life care coordination when necessary.
  • Supervise and guide direct reports, fostering a supportive and educational environment.
  • Adhere to all professional practice regulations and maintain confidentiality regarding all client and company information.
  • Participate in ongoing training and quality improvement initiatives.
  • Communicate effectively with supervisors and team members regarding workload and priorities.
  • Complete all required training and health screenings in a timely manner.
  • Adapt to changing business needs and perform additional duties as assigned.

Qualifications:

  • A minimum of five years of progressively responsible experience in physical therapy.
  • At least two years of experience working with elderly or frail populations in a clinical environment.
  • Strong clinical knowledge and skills to address the complex needs of older adults.
  • Demonstrated leadership abilities with experience in mentoring and developing team members.
  • Excellent organizational and communication skills, particularly in settings involving seniors and their families.
  • Ability to work independently and make informed decisions in high-pressure situations.
  • Commitment to serving diverse populations and enhancing the quality of life for vulnerable seniors.

Education Requirements:

  • Bachelor's degree in Physical Therapy or equivalent.
  • Master's degree or Doctorate in Physical Therapy preferred.
