Overview:

The Licensed Physical Therapist plays a crucial role in delivering restorative and rehabilitative services to clients both at the facility and in their homes. This position requires close collaboration with medical professionals and rehabilitation teams to enhance client independence and safety while focusing on functional recovery.

Key Responsibilities:

  • Conduct comprehensive assessments to identify the rehabilitation needs and objectives of clients, actively participating in interdisciplinary team meetings to formulate care strategies.
  • Implement and document therapeutic interventions as per physician directives and care plans, including but not limited to therapeutic exercises, muscle re-education, and functional training.
  • Supervise and mentor Therapy Assistants, providing essential training for the execution of Functional Maintenance Programs.
  • Educate clients, their families, and staff on the effective use and maintenance of therapeutic equipment.
  • Monitor and manage equipment maintenance and ensure compliance with safety standards.
  • Maintain open communication with team members and promptly report any changes in client conditions.
  • Facilitate smooth transitions of care across various settings, ensuring continuity and quality of services.
  • Support enrollment initiatives in collaboration with the marketing team.
  • Participate in end-of-life care planning and coordination.
  • Ensure timely and accurate completion of all required documentation and training.
  • Adhere to company policies and engage in quality improvement activities.

Qualifications:

  • A minimum of five years of progressive experience in physical therapy.
  • At least two years of experience working with elderly or frail populations in a clinical environment.
  • Strong knowledge of physical therapy practices tailored to complex client needs.
  • Proven leadership skills with experience in mentoring and developing team members.
  • Exceptional communication and organizational skills, particularly in multidisciplinary settings.
  • Ability to work independently and manage multiple priorities effectively.

Education:

  • Bachelor's degree in Physical Therapy or equivalent.
  • Master's degree or Doctorate in Physical Therapy preferred.
