Job Summary

We are seeking a highly skilled and experienced Clinical LPN/RN to join our team at Southeast Medical Group. As a Clinical LPN/RN, you will play a vital role in providing clinical support functions and professional nursing care to patients.

Key Responsibilities
  • Assist Providers and Clinical Staff: Identify high-risk patients and assist in the development of personalized contingency plans.
  • Conduct Evaluations and Patient Education: Perform evaluations and patient education nurse visits for chronic disease states, providing clear instruction on treatment, medications, and self-management goals.
  • Triage Incoming Calls: Respond to incoming calls and perform outbound calls regarding band stratification allocation.
  • Review Patient Supportive Care: Assess patient supportive care at home, social factors, and concerns, including durable medical equipment, home health, and other resources.
  • Communicate with Hospitals and Skilled Nursing Facilities: Communicate with hospitals and skilled nursing facilities regarding inpatients, ensuring effective communication with physicians and staff.
  • Access Medical Records: Access designated local hospital physician portals to retrieve medical records when necessary.
  • Assist with Communication: Assist providers with communication to the Care Management Team and document in the electronic medical record when appropriate.
  • Participate in Daily/Weekly Huddles: Participate in daily/weekly huddles and Care Coordination meetings, ensuring current patient information and status are up-to-date.
  • Perform Clinic Patient Wound Care: Perform clinic patient wound care on stable conditions, assessing and documenting appropriately.
  • Multi-Task and Collaborate: Demonstrate the ability to multi-task and work with multidisciplinary teams and individuals.
  • Perform Nursing Care Duties: Perform all nursing care duties within the scope of a Licensed Practical Nurse/Registered Nurse, including injections, procedures, phlebotomy, and diagnostic testing.
Requirements
  • Licensed Practical Nurse/Registered Nurse: Current license in good standing to practice in the state of employment.
  • Georgia State Practical Nurse/Registered Nurse: Current licensure in the state of Georgia.
  • CPR Certification: Current CPR certification.
  • Electronic Medical Records: Ability to work efficiently with electronic medical records.
  • Medical Terminology: Effective use of proper grammar, spelling, and medical terminology.
  • Emergency Situations: Ability to react calmly and effectively in emergency situations.
  • Related Experience: Two years of related LPN/RN experience in a physician's office, clinical, or hospital setting.
  • IV Certification: Current IV certification.
  • Communication Skills: Effective listening and communication skills.
  • Infusion Experience: Experience with infusion therapy.
Preferred Qualifications
  • Three or More Years of Experience: Three or more years of related LPN/RN experience in a physician's office, clinical, or hospital setting.
  • Managed Care Knowledge: Knowledge of managed care and referral processes.
  • Patient Education Experience: Experience related to patient education.
  • Xray Experience: Experience with x-ray procedures.
  • Chronic Disease Knowledge: Proficient knowledge of chronic diseases, especially COPD/asthma, CHF, IHD, and diabetes.
Physical and Mental Requirements
  • Lifting and Pushing: Ability to lift up to 50 pounds and push or pull heavy objects using up to 50 pounds of force.
  • Sitting and Standing: Ability to sit for extended periods and stand for extended periods.
  • Fine Motor Skills: Ability to use fine motor skills to operate office equipment and/or machinery.
  • Verbal and Written Instructions: Ability to receive and comprehend instructions verbally and/or in writing.
  • Problem-Solving: Ability to use logical reasoning for simple and complex problem-solving.
  • Travel: Ability to travel to multiple locations to support business needs as required.
