LPN Hospice Care Specialist

We are seeking a compassionate and skilled LPN to join our Hospice Home Care Team in Schenectady/Albany Counties. This is a full-time day shift position, Monday through Friday, 8:00am to 4:00pm. As an LPN on our team, you will provide nursing and personal care services to patients and families, working closely with our RN Supervisor and RNs to develop and implement comprehensive plans of care.
About the RoleThis is an ideal opportunity for someone who enjoys working as part of a collaborative team, providing end-of-life care in a patient's home environment. You will have the opportunity to work with a multidisciplinary team, including RNs, Aides, medical directors, social workers, chaplains, complementary therapists, and volunteers.
Responsibilities- Coordinate with RN Supervisor and RNs to provide nursing and personal care services
- Develop and implement comprehensive plans of care to support patients and families
- Work collaboratively with the healthcare team to ensure high-quality patient care
- Valid NYS LPN License
- Minimum 1 year LPN experience
- Prior hospice/home care experience preferred, but not required
- Strong computer skills
- Organization and communication skills, with ability to work as part of a team
The estimated salary for this position is $24.00-$30.10 per hour, based on experience, skills, and education. We offer a competitive benefits package, including health insurance, retirement plan, and paid time off.
